In this regard, the aim of this study was to determine the prevalence of depressive symptoms and to evaluate the association between the number of daily working hours and depressive symptoms in Peruvian residents. Methods: Analytical cross-sectional study that used the database of the National Survey for Resident Physicians-2016, a voluntary survey issued virtually by the National Council of Medical Residency of Peru to physicians who were undertaking their residency in Peru. The presence of depressive symptoms was considered as having obtained a score ≥3 with the Patient Health Questionnaire-2 scale. The number of hours worked each day was collected through a direct question. To assess the association of interest, prevalence ratios (PR) and their 95% confidence intervals (95% CI) were calculated using crude and adjusted Poisson regressions with robust variance. Results: The responses of 953 residents (41.3% women, mean age: 32.5 years) were evaluated, 14.6% of which presented depressive symptoms. In the adjusted analysis, it was found that the prevalence of depressive symptoms increased for each additional hour worked (PR = 1.11; 95% CI, 1.04-1.17).

Thus, the aim of this study was to conduct a scoping review to describe the characteristics and quality of CPGs for Depression in adults.

Si bien esta RM ha implicado un avance en la salud ocupacional peruana, presenta ciertas falencias con respecto al tamizaje de tuberculosis, que podrían estar impidiendo un diagnóstico certero en la población vulnerable.
- PublicaciónSólo datosEffects of dog-assisted therapy in adults with dementia: a systematic review and meta-analysis(BioMed Central Ltd., 2019-01) Zafra-Tanaka, Jessica Hanae; Pacheco-Barrios, Kevin; Andree Tellez, Walter; Taype Rondán, ÁlvaroBackground: Dog-assisted therapy (DAT) is a non-pharmacological intervention based on the interaction between patients and dogs, which has been proposed to help adults with dementia. However, evidence to support it is lacking. Thus, we aim to evaluate the effects of DAT on this population and to assess the certainty of the evidence of the RCTs estimates. Methods: A systematic search was performed. We included randomized controlled trials (RCTs) and quasi-experimental (QE) controlled studies published up to March 2018, which evaluated the beneficial and deleterious effects of DAT in adults with dementia. Mean differences (MD) or standardized mean differences (SMD) and their 95% confidence intervals (95% CI) were calculated and random effects meta-analyses were performed. Certainty of evidence was assessed for RCTs estimates using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) methodology. The study protocol has been registered in PROSPERO (CRD42018090434). Results: Ten studies (six RCTs and four QE controlled studies) were eligible for inclusion. Meta-analysis of RCTs showed no effect of DAT in daily life activities (SMD: 0.16; 95% CI: -0.80 to 1.12), depression (SMD: -0.45; 95% CI: -2.81 to 1.91), agitation (SDM: -1.12; 95% CI: -2.67 to 0.43), quality of life (SDM: 0.16; 95% CI: -0.41 to 0.73), and cognitive impairment (SDM: -0.52; 95% CI: -1.33 to 0.30), but it found a beneficial effect in apathy (1 study, n = 37, MD: 1.81; 95% CI: 1.26 to 2.36). All outcomes had a very low certainty of evidence according to GRADE methodology. Conclusions: RCTs evidence of very low certainty suggests that, in adults with dementia, DAT has no effect in daily life activities, depression, agitation, quality of life, and cognitive impairment, although one small study found an apparent beneficial effect in apathy. More well-designed and correctly reported studies are needed in order to provide a conclusion. Trial registration: CRD42018090434 (PROSPERO).

Dicho juramento ha sido actualizado en distintas ocasiones por la Asociación Médica Mundial (AMM), una organización internacional que busca representar a la profesión médica. En el presente artículo repasamos la historia de estas actualizaciones.

Posteriormente, la Declaración de Ginebra fue enmendada en las Asambleas de la AMM en 1968, 1983 y 1994; y fue revisada editorialmente por el Consejo de la AMM en 2005 y en 2006.

Colegio Médico del Perú, 2018-01) Taype Rondán, Álvaro; Alarcon-Ruiz, Christoper A.; Zafra-Tanaka, Jessica Hanae; Rojas-Villar, Feeder J.Objetivo: Evaluar los factores asociados al ingreso económico, carga laboral y clima laboral en un grupo de médicos generales jóvenes de Perú. Materiales y métodos: Estudio transversal analítico en médicos generales que asistieron a un evento del Colegio Médico del Perú. Durante dicho evento, se realizó una encuesta para recoger las características de sus dos últimos empleos. Para evaluar las asociaciones de interés se calcularon odds ratios crudos y ajustados (ORa) usando modelos de regresión logística de efectos mixtos. Resultados: Se analizaron 332 empleos reportados por 221 médicos. El ingreso económico bajo (S/ 16,00 por hora o menos) fue menos frecuente para empleos en el Ministerio de Salud – MINSA (ORa: 0,38) o en el seguro social – EsSalud (ORa: 0,09) en comparación con el sector privado, pero más frecuente en mujeres (ORa: 1,94), y en quienes realizaban una labor asistencial (ORa: 4,31). El referir mucha carga laboral fue más frecuente en aquellos con ≥ 30 años (ORa: 2,72), y para empleos en el MINSA (ORa: 3,13) o EsSalud (ORa: 7,98) en comparación con el sector privado. El referir un clima laboral inadecuado no presentó factores asociados.

Objective: To assess mortality in patients with acute kidney injury undergoing hemodialysis (HD) and its associated factors in a developing country setting. Methods: Retrospective study. Demographic, clinical, and mortality variables were collected from patients who presented AKI and underwent HD between January 2014 and December 2015 at a national reference hospital in Lima, Peru. Risk ratios (RR) and 95% confidence intervals (95%CI) were estimated through Poisson regressions.

To estimate the mortality attributable to diabetes mellitus (DM) as recorded by Peru’s Ministry of Health and its association with the human development index (HDI). Methods. This was an ecological study based on a secondary analysis of death records belonging to the Ministry of Health for the period from 2005 to 2014. A death was considered attributable to DM if the underlying cause of death given in the death record was DM. Mortality attributable to DM has been presented descriptively and in terms of geospatial analyses, and Spearman’s rho was used to test for an association between the difference in the mortality attributable to DM (between 2005-2006 and 2013-2014) and the HDI in Peru’s various departments.

Moreover, those who migrate from farther places to perform their SHS could have an increased risk. The objective of this study was to evaluate the association between place of origin and the presence of depressive symptoms, in health professionals performing rural social health service (SHS) in Ancash, Peru. Methods: This was a cross-sectional study. During April 2015, a survey was applied to health professionals performing SHS in the Peruvian Ministry of Health (MINSA) facilities in Ancash. The main outcome was the presence of depressive symptoms, defined as a score ≥2 points in the Patient Health Questionnaire-2. The main exposure was the place of origin, defined as the place where the subjects completed their undergraduate professional studies (Ancash, Lima city or others). Poisson regressions with robust variance were performed to calculate crude and adjusted prevalence ratios (PR and aPR) and their 95% confidence interval (95%CI). Results: From 573 health professionals performing their SHS in MINSA in Ancash, 347 were included in the study. The mean age was 27.2±4.5 years, 78.7% were women, and 14.7% scored positive for depressive symptoms. Those who had completed their undergraduate professional studies in Lima city had a higher prevalence of presence of depressive symptoms compared to those who did in Ancash (aPR=2.59, 95%CI=1.23–5.45). Conclusions: Those who completed their undergraduate professional studies in Lima had a higher prevalence of depressive symptoms than those who did in Ancash. Possible explanations include the difficulty in visiting family and friends, acculturation, and lack of Quechua language proficiency.

Sujetos y métodos: Estudio transversal analítico usando los datos obtenidos por la Encuesta Nacional para Médicos Residentes 2016 (ENMERE-2016), realizada virtualmente durante junio de 2016. Población: los médicos que realizan su residencia en alguna universidad de Perú. Muestra: los residentes que voluntariamente participaron en la ENMERE-2016. Desenlace de interés: autorreporte de haber publicado al menos un artículo científico. Se recogieron variables sociodemográficas, de la universidad, de la sede, de la residencia, de la percepción del residente y de síntomas depresivos. Para evaluar los factores asociados a haber publicado algún artículo científico se calcularon razones de prevalencia crudas y ajustadas con sus intervalos de confianza al 95%.

Sin embargo, en Perú, un país con incipiente producción científica, estos trabajos no suelen publicarse en revistas científicas indizadas, lo cual limita drásticamente su visibilidad e impacto. Objetivo: Caracterizar la producción científica de estudiantes de pregrado de la Facultad de Medicina Humana de la Universidad de San Martin de Porres, durante el periodo 2005 - 2016. Métodos: Estudio bibliométrico. Se realizó una búsqueda bibliográfica en los portales web de las bases de datos SCOPUS, SciELO, LILACS y LIPECS, usando términos que identificaran a la FMH-USMP. Se seleccionaron aquellos artículos que tuvieron a algún estudiante de la Facultad de Medicina Humana de la Universidad de San Martin de Porres como autor. Posteriormente, se recolectaron las características de interés para cada artículo. Los análisis estadísticos fueron hechos con el software estadístico Stata v13.

However, in Peru, a country with scarce scientific production, these works are not usually published in indexed scientific journals, which drastically limits their visibility and impact. Objective: To characterize the scientific production of undergraduate students of the Human Medicine School of University of San Martin de Porres, during the period 2005-2016. Methods: Bibliometric study. A bibliographic search was carried out in the web portals of the databases SCOPUS, SciELO, LILACS and LIPECS, using terms that identify the FMH-USMP. We selected the articles that had a student of the Human Medicine School of University of San Martin de Porres as author. Subsequently, the characteristics of interest for each article were collected. The statistical analysis was carried out with the statistical software Stata v13. Results: A total of 160 scientific publications with student participation were found. On average, 1.63 % of the students of the Human Medi cine School of University of San Martin de Porres participated in some scientific publication yearly. Among the articles evaluated, 39 (24.4 %) were published in journals indexed to Scopus, 31.3 % and 55.6 % had as their first author and correspondent author a student of the Human Medicine School of University of San Martin de Porres, respectively, while 66.9 % had the participation of students from one of the scientific societies of medical students (SOCEM) of the Human Medicine School of the University of San Martin de Porres. In addition, 65.0 % are original articles, among which 60.6 % were descriptive observational studies.

Métodos: Estudio bibliométrico. Se identificaron publicaciones científicas sobre diabetes que tuvieran algún autor con filiación peruana, en las bases de datos Web of Science Core Collection (WoS) y Scopus, entre 1996 y 2015. Se extrajeron manualmente las variables de interés. Se presentaron los resultados descriptivos y las redes de colaboración. Resultados: Se encontraron 81 publicaciones, el 75,3% fueron artículos originales, de las cuales ninguno evaluó intervenciones en salud pública y 60,7% recibieron financiamiento extranjero. El 55,6% tuvieron algún autor afiliado a la Universidad Peruana Cayetano Heredia (UPCH), y el 65,4% algún autor extranjero. Se puedo distinguir cinco conglomerados de redes de colaboración internacional, todos con participación de la UPCH.

Methods This was a cross-sectional secondary analysis that used data from the Peruvian Medical Residents National Survey 2016 (ENMERE-2016). The outcome of interest was workplace violence, including physical and verbal violence, which were categorized according to the perpetrator of violence (patients/relatives and worker-to-worker). Primary exposure was the medical specialty, categorized as clinical, surgical, and other specialties. To evaluate the associations of interest, we estimated adjusted prevalence ratios (PR) with their respective 95% confidence intervals (95% CI) using Poisson regression models with robust variances. Results A total of 1054 Peruvian medical residents were evaluated. The mean age was 32.6 years and 42.3% were female. Overall 73.4% reported having suffered of workplace violence sometime during the residency, 34.4% reported violence from patients/relatives, and 61.1% reported worker-to-worker violence. Compared with clinical residents, surgical residents had a lower prevalence of violence from patients/relatives (PR: 0.71; 95% CI: 0.59–0.87), but a higher prevalence of worker-to-worker violence (PR: 1.11, 95% CI: 1.01–1.23).
